Welcome to Granada Dental!  Please take a moment to complete/update your information.  We ask that you put aside a few minutes of uninterrupted time to complete the forms because as part of our security measures, the page will time out if there is inactivity.  Therefore, please have your emergency contact, insurance, and medical and dental information handy.  Should you have a list of medications that you take, you do not need to list it here but we ask that you bring it with you to your appointment and we will scan it in your records.

Lastly, your first visit may only include a comprehensive examination and x-rays with Dr. Nazari.  Teeth prophylaxis (cleaning) is not usually scheduled because our hygienist Shaina has a separate schedule.
